Actor Hayden Panettiere died from an accidental drug overdose involving fentanyl, according to the Greenville County Coroner’s Office and reported by CBS News.

Emergency responders found Panettiere at a residence in Greenville, South Carolina, on August 16. She was pronounced dead despite advanced cardiac life support efforts.
An autopsy determined that the death resulted from the toxic effects of fentanyl, 4-ANPP, alprazolam, methocarbamol and quetiapine. The coroner’s office found no signs of trauma and classified the death as accidental.
