An Army report has identified “systematic failures” behind the 2025 midair collision between an American Airlines jet and a Black Hawk helicopter near Washington that killed 67 people.
According to the report obtained by The Associated Press, breakdowns across multiple layers of airspace conflict management contributed to the accident. The report’s author said two failed radio transmissions were the most significant contributing factor because they prevented the helicopter crew from properly identifying Flight 5342.
Families of those killed are urging Congress to strengthen aviation safety rules, including wider use of collision-avoidance technology to help pilots detect nearby aircraft.
The House and Senate have passed separate aviation safety bills, with lawmakers now working toward a final compromise.
