The US Supreme Court has blocked President Donald Trump’s administration from suspending hormone therapy and other gender-affirming treatments for transgender inmates in federal prisons, delivering a rare setback to the president on transgender policy.

Trump signed an executive order on his first day back in office barring treatments intended to align prisoners’ physical appearance with their gender identity. The Bureau of Prisons subsequently prohibited hormone therapy, surgery and certain social accommodations while permitting psychological services.

Three inmates challenged the policy, arguing it would end established treatment for nearly 1,000 prisoners. Lower courts had temporarily blocked the restrictions, with an appeals court finding that the administration had likely failed to adequately assess its previous experience providing such care.

The administration argued that security and prison management concerns justified the change. The American Civil Liberties Union disputed that rationale, citing the absence of reported security incidents linked to the treatment over three decades.
