National Security Adviser Mike Waltz has taken responsibility for the security breach, which occurred from unknowingly adding journalist Jeffrey Goldberg to a group chat where high-ranking officials in the Trump administration discussed military strikes in Yemen. Waltz admitted to creating the group and called the situation "embarrassing."

Waltz, unable to explain how Goldberg was added to the chat, suggested a technical glitch, with a staff member’s contact possibly being mixed up, in an interview with Fox News's Laura Ingraham.
